Student Accident Reports
If a student is injured during a fieldwork-related activity/task, please contact the course instructor on the day that it occurs so that they are informed. Complete the UofT Accident Reporting Form linked here and email it to rss.otclined@utoronto.ca. UofT will follow up with further instructions.
Travel Allowance Forms
If a placement is listed as “car required,” please complete and submit the Travel & Relocation Allowance Form on or before the Monday following the final day of your fieldwork placement. Follow the format of the Sample Fieldwork Mileage Tracking Form and include with your submission.
Additional Forms
Complete these forms when you have a specific request related to a fieldwork course that is not related to disability-related accommodations.
